Was there a recent change in which feeds without new items appear on the list? Is there a setting where I can only see feeds with new items?

Option buttons are on the bottom left: All, Unread, and Focus. Sounds like you have All selected but would rather Unread (this will show only feeds with new items). Focus (the green dot) will appear if you’ve “trained” a story with a thumbs-up using the tags, titles, or authors on the item (likewise red items (thumbs-down) will not show, but the feed header will display an indication the feed has hidden items).
